Why do you want so much personal information - real name, date of birth, email address, blog/twitter/FB/lj? What are you planning to do with the information?
Why are real name and date of birth required fields? This makes me quite uncomfortable. Maybe others feel free to hand over personal information to a total stranger but I don't understand why anyone would need it.
